  1. Click here to Pay for your Credit Report Processing Fee.
  2. Copy of current pay stubs for a one (1) month period for all adult household members
    • If self-employed, Profit & Loss Statement that is no more than 90 days
    • For Social Security will need award letter or bank statement showing direct deposit
  3. Copy of 2018 and 2017 Federal Income Tax Returns, including W2 forms (for any year not filed, provide a written explanation as to why)
  4. Copy of last three (3) months Bank Statements (all accounts, all pages)
  5. Reviewed, signed and dated, “Pre-Purchase Counseling Agreement and Disclosure”
  6. Complete, signed and dated, “Credit Authorization / Pre-Qualification Form”
  7. Complete, signed and dated, “Homebuyer Program Eligibility Questionnaire”
  8. Complete, signed and dated, “No Income Certification,” for household member and co-borrower (if
    applicable

MORTGAGE ASSISTANCE APPLICATIONS

Intake Forms:

  • NHS Intake: Intake, Budget & CFPB Questionnaire, completed, signed and dated
  • Disclosure Agreement, signed and dated
  • Privacy Policy, signed and dated
  • Authorization for Servicer, completed, signed and dated

Supporting Documentation:

  • Most recent Mortgage Statement
  • Most recent Proof of Income, for any borrowers and spouse, (whichever is applicable)
    • Two (2) most recent Pay Stubs
    • Award Letters (Social Security, Pension, Disability, Unemployment, etc.)
    • Profit & Loss Statement (Year-to-Date), if self employed
  • Two (2) months most recent Personal Bank Statements (ALL PAGES, even if blank, Online Transaction
    Printouts are NOT acceptable.
    However you should be able to print out an actual statement online.)

    • If self-employed, please provide three (3) months recent Business Bank Statements, in add
  • Most Recent Personal Federal Tax Returns & W-2s/1099s (1040, ALL PAGES/SCHEDULES, Please
    SIGN & DATE PAGE 2)
  • Hardship Letter including:
    • Start Date of Hardship, Financial Impact $$$ of the hardship (e.g. household income decre
      from $4,000 to $3,000 per month or expenses increased from $3,000 to $3,800 per month)
  • Current Property Tax Statement, even if insurance is paid through escrow/impound account
  • Current Homeowner’s Property Insurance Statement (Declaration Page)
  • Copy of most recent Loan Modification, if applicable
  • Notice of Default and/or Notice of Trustee Sale, if applicable

HOME REPAIR APPLICATIONS

ITEMS NEEDED FOR REVIEW – PLEASE PROVIDE COPIES WE CAN KEEP

  5. Copy of Property Taxes
  6. Copy of Mortgage Billing Statement for all loans
  7. If your original loan has been modified please provide a copy of the modification agreement
  8. Please provide a letter for what improvements you need for your house
  9. Read, Sign and Date Disclosures of Programs and Services Form
  10. Complete, Sign and Date Credit Authorization Pre-Qualification Form
  11. Complete, Sign and Date Homebuyer Program Eligibility Questionnaire
  12. Complete, Sign and Date No Income Certification For Household Member or Co-Borrower (if needed)
